Instructions

To attend the Team Hammer event, print out the registration pdf (above) and complete all forms.

Completed forms with payment must be received in our office seven days before the event to avoid post-entry fee of $50.

Payment must be received with registration forms. Send cashier’s check, money order or credit card authorization. No personal checks, please.

When we receive all your completed original forms (FAXed copies not acceptable) and required payment we will process your application and send you a letter to confirm your reservation.
